Fitbit type thing as other have mentioned.
However, I always assumed it was some sort of padding; that's the arm he got drilled on in the Niners SB when he was going to the end zone. His arm / tricep area was damn near turning blue by the end of that game; or at least starting to. I always figured he just wears a bad there now as maybe it's still sensitive if hit the wrong way.
padding looks like it's in the exact spot here I think the defender's knee or helmet tagged him with in the tricep on the hit where on the mic'd up Pat says something like "that's a good as hit though, 21" (or whatever the players number was). [Reply]
Now this is mindblowing right here. Only 4 seasons and he has already passed guys like Rodgers, Brees, and Manning in terms of win probability impact in the playoffs. The clutch factor with Patrick is off the charts.
